LIFE STEPS to Weight Loss and Better Health is a year long, in-depth lifestyle change program aimed at improving ones overall health and wellness.

  • Decreasing your risk of developing type 2 diabetes, heart disease, or other chronic disease, through gradual weight loss and increased activity.
  • For those participants who already have diabetes, heart disease or another chronic disease, this program will help them to make the necessary lifestyle changes needed to better control their chronic disease.

Participants will get the support of a Registered Dietitian, a small group of peers, and tools that will help them develop achievable, realistic, and long-term lifestyle changes. They will learn strategies to break unhealthy habits, eat well, be physically activity, and improve problem-solving and coping skills.

Participants meet for 26, one hour supportive small group sessions, over the course of a year. This program is split into two phases in order to address your individual needs one at a time.

Phase 1: 16 weekly sessions – meal planning, coming up with solutions to your barriers for making changes, and general lifestyle education.

Phase 2: 4 bi-weekly sessions, and 6 monthly sessions – continued support and additional resources, based on the needs of the group.

Registration required.

Call our Diabetes & Nutrition Education office at  651-430-8715 for more information.

(Program cost is $300 for the entire year.)
